Resource List for Employees/Families Affected By Hurricane Katrina

SAC James Bernazzani has authorized administrative leave until October 1, 2005. This leave is to cover personal and family issues as a result of Hurricane Katrina. If administrative leave is extended past October 1, 2005, employees will be notified as soon as possible. Employees should continue to contact the Shreveport RA daily (weekdays). Decisions regarding TDY assignments outside the division are still pending, with final approval through FBIHQ.

SAC Bernazzani has prepared a letter (09/10/05) to all New Orleans Division employees that is available through each FBI field office.

2. Employee Assistance Program (EAP), Victim/Witness Assistance, and Administrative Services Division are all ready to provide assistance in the following areas:

3. Officials are letting residents back into Jefferson Parish between 6 AM - 6 PM (9/5/05) with photo ID showing they live in the area. Officials suggest residences handle personal matters in Jefferson Parish and then leave the area.

4. All employees that want to check the status of their homes, can report to Camp Villere, FBI Firing Range (Slidell) to get assistance. Camp Villere is located off Interstate 12, Exit 80, Northshore Blvd., Slidell. EAP personnel, in Shreveport can provide information on homes that have been checked.

5. View WWW.FBI.GOV for updates. There is an icon for New Orleans Division employees. This site will be updated when new information is available.

9. Emergency Loans

Justice Federal Credit Union has put together an emergency assistance package to provide relief to our members and their family members who have been affected by Hurricane Katrina in Alabama, Florida, Mississippi, and Louisiana. Effective immediately, the following emergency assistance plan is available to all affected members, their family members, and potential members.

* Personal loans - Amounts up to $2,500 at 0% APR* with a maximum of 12 months to repay. Amounts from $2,501-$10,000 at 5.00% APR* with a maximum of 36 months to repay.

* Auto loans - 4.5% APR*, for up to 60 months.

* Deferred payments - up to 90 days on all new and existing auto, personal, and home equity loans, including emergency assistance loans.

* Deferred mortgage payments - up to six months deferred payments on uninhabitable homes financed by Justice FCU.

All emergency assistance loans will be given top priority and processed before other loans. You may take advantage of payment deferral and/or apply for these loans by contacting us at (800) 550-5328, Monday - Friday from 8:30 a.m. - 8:00 p.m. (EST).

10. The FBI Agents Association (FBIAA), which has established a Hurricane Katrina Relief Fund, to assist all Bureau employees who are recovering from the long-term effects of the storm. Members of the FBI Family who wish to make a voluntary contribution to help their fellow employees recover from this devastating event may do so by making checks payable to ""FBIAA Hurricane Katrina Relief Fund"" and sending them directly to the FBIAA national office at PO Box 250, New Rochelle, NY 10801.The funds collected will be distributed among all Bureau employees who suffered loss of personal property, damage to their homes, and other unforeseen losses. Insurance will not cover all the costs that a situation such as this has thrust upon our fellow employees in the states of Alabama, Florida, Louisiana and Mississippi. Requests for assistance can be made through the EAP representative at the Shreveport Resident Agency Command Post, 318-221-8439.

11. The St. Tammany School System reports that the goal is to have students report on October 3, 2005. Final decisions about reopening the St. Tammany School System will be communicated through the local media and www.stpsb.org. As information is received regarding other school systems, it will be posted to the website.

12. New Orleans Division is now capturing Time & Attendance (T&A) records in the Shreveport RA. Resident Agencies (RA) should continue to record the T&A in the RAs.

13. SAC Bernazzani, will be at Camp Villere on Tuesday, Sept. 20, at 10:30 am. to meet with New Orleans Division employees in the New Orleans area. Any employees that are available, are requested to attend.

14. SAC Bernazzani will have an conference/lunch for New Orleans Division employees located in the Houston area. The Houston Division has arranged a meeting of the New Orleans employees, Wednesday, Sept.21, from 11:00 am - 1:00 pm, at the Minute Maid Ball Park Club House. Point of contact is Houston Division EAP Coordinator Theresa L. Millard, 713-693-5220.

15. Insurance Information:

State Farm

- To initially report a claim, call 1-800-732-5246.

- If you have already started the claim process, contact State Farm again to update & verify your contacts numbers.

- Once you are " qualified" as a policy holder, and if your house in within the affected areas, they are making $25,000 advance on your policy benefits. Proper identification needs to be presented by the policy holder to pick up the check.

- They have an Advance Payment Office in Shreveport, located at 1915 East 70 th Street, where they are giving out the checks. No phone number was on record in their database.

- They have advised that there are 174,000+ affected policy holders in Louisiana. The company actively has tried to contact policy holders, but many zip codes have no service, and the claims adjusters have not been able to get into the worst areas of the area.

Allstate

- To initially report a claim, call 1-800-255-7828. It is advisable to call this number initially to get the process started.

- They have three claim processing centers for those in Louisiana:

- Home Depot @ 18139 Highland Road, Baton Rouge

- Home Depot @ 10200 Courtesy Blvd., Baton Rouge

- Wal-Mart @ 10750 West View Drive, Houston, Texas which has been set up for those evacuees from Louisiana

USAA

- To initially report a claim, call 1-800-531-8111. All you need is your SS# or policy number when you call. Call this number to get the process started.

- Note: they have a task force set up at Kiesler AFB to inspect the damage, etc.

16. Contact Victim Specialist Sarah Abdullah, 504-329-9854 regarding access to FBIAA funds.

17. Due to Hurricane Rita, the meeting with USAA Insurance Representatives Brooks Wilson and Tim Vogt has been cancelled. USAA representatives have evacuated to Jackson, Mississippi. Please advise any employees affected.

18. There will be an All Employee Conference for New Orleans Division headquarters city personnel on Monday, October 3, 2005 at 11:00 am at the Covington Country Club, 400 Country Club Drive, Covington. Please contact your supervisor for details.

19. Arrangements have been made for the All Employee Conference to be broadcast on a conference call bridge. Please contact the Shreveport RA for details on accessing the conference call.

20. USAA will have representatives available after the All Employee Conference at the Covington Country Club after the All Employees Conference.

21. SAC, New Orleans has made decisions regarding employees that requested TDY assignments. Please contact your supervisor for details.
